STUDIO AND ON-DEMAND TERMS AND CONDITIONS

STUDIO CANCELLATION POLICY

Cancellations must be done through the STUDIO FORMA website. We do not accept cancellations by telephone or email.

 All group classes require a 24 hour cancellation notice. You may cancel your reserved classes for any reason without penalty up until 24 hours prior to the scheduled class. A credit will be applied to your account to use for another time.

Rescheduling or cancelling a reserved class after the 24-hour period is a late cancellation and you will forfeit the class. No refunds.

Late Cancellation Fee: $10

No Show Fee: $25 (No show is canceling 1 hour prior to your class time or not showing up for class)

 Classes start promptly at the scheduled time, so please arrive 10 min before class to get settled in. For the safety of our clients and staff, the door will lock at the start of class, so please do not be late!

We reserve the right to cancel the class at any time and you will receive a credit on our behalf. 

PURCHASE & REFUND POLICY

All Packages and Merchandise are Final Sale. 

 Packages may be transferable to another client with approval from Studio Forma by emailing jess@studioforma.com 

STUDIO ETIQUETTE

We are a shoeless facility and we ask kindly that you remove your shoes before stepping into the mat area.

While we understand that you may need to keep your phone by you for an important call, we ask to please take any calls in the lobby or step outside to not disrupt the class in session. If you do not NEED your phone, please keep it with your other personal belongings in the designated area.

We take pride in the cleanliness of the space and we ask that you wipe down your area after class and tidy up. It just takes a moment and is very helpful to us.

One our favorite things about the studio is the community and the chit chat. Before and after class is when that should take place so the trainer can successfully guide everyone through the routine and stretch.

ON DEMAND TERMS & POLICY

1. Subscription & Billing

  • Subscriptions are available on a monthly or yearly auto-renew basis.

  • Your chosen payment method will be charged automatically at the start of each billing cycle.

  • You agree to keep your billing information current and accurate. If payment is declined or cannot be processed, your subscription may be suspended or deactivated until updated payment information is provided.

  • False or inaccurate account information (including billing details) is prohibited and may result in immediate suspension or termination of your account.

  • By subscribing, you authorize Studio Forma (and its designated third-party payment processors) to charge your selected payment method for recurring subscription fees, applicable taxes, and any other agreed-upon charges.

  • Third-party processors: Payments are processed through secure third-party services. Studio Forma does not store full credit card information and is not responsible for errors, delays, or issues arising from the payment processor.

  • Refunds are not provided for partially used billing periods. Refunds are only granted in cases of duplicate billing or verified technical error.

2. Cancellation

  • You can cancel your subscription at any time through your account settings.

  • Upon cancellation, you will retain access until the end of your paid term.

  • To avoid being charged for the next cycle, cancellation must be completed before the renewal date.

3. User Responsibilities

  • Studio Forma OnDemand content is for personal, non-commercial use only.

  • You may not copy, download, distribute, or share content outside of your account access.

  • Access credentials are individual — accounts may not be shared.

4. Account Security

  • You are responsible for maintaining the confidentiality of your username, password, and account details.

  • You agree to notify Studio Forma immediately of any unauthorized use of your account or suspected security breach.

  • Studio Forma is not liable for any loss or damage resulting from failure to safeguard your account information.

5. Intellectual Property

  • All Studio Forma OnDemand content, including but not limited to videos, written materials, logos, graphics, and trademarks, are the intellectual property of Studio Forma.

  • You are granted a limited, non-exclusive, non-transferable license to access and use the content for personal purposes only.

  • Any unauthorized reproduction, distribution, modification, or commercial use of Studio Forma content is strictly prohibited and may result in legal action.

6. Health & Safety Acknowledgment

  • By participating in Studio Forma OnDemand, you acknowledge that physical exercise carries inherent risks.

  • You represent that you are in good physical condition and have consulted with a healthcare professional before starting any exercise program.

  • You agree to assume full responsibility for your participation and waive any claims against Studio Forma, its instructors, employees, and affiliates arising from injury, illness, or damages related to participation.

7. Indemnification

  • You agree to indemnify and hold harmless Studio Forma, its instructors, employees, contractors, and affiliates from any claims, damages, or expenses resulting from your use of Studio Forma OnDemand.

8. Media Release

  • By using Studio Forma OnDemand, you consent to the use of class recordings and related media for marketing and promotional purposes. Personal identifying information will not be shared without consent.

9. Governing Law

  • These terms are governed by and construed in accordance with the laws of the State of Illinois.
